HIMSS 2012: STEM Transition of Care Pilot with Denver Health, Division of Hematology-Oncology [DHC] and Jefferson County Health Care Program for Children with Special Needs [JCHCP]: Live demo steps embody transition-of-care scenarios for Mary Sample, an 11-year-old with leukemia who lives in Colorado and visits grandparents in Virginia and how all the care information will travel with her as she visits her interim pediatrician in Virginia.
Arizona Department of Health Services(ADHS): Direct Immunization Pilot: Enabling ADHS with Direct Transport Specification and Interfacing and Integrating with the evolving Arizona HISP Marketplace.
Transition of Care Pilot in Arizona County Hospital: Maricopa Integrated Health Systems (MIHS): Enable MIHS with Direct Transport to receive Transition of Care CCDs between the primary and the referring physicians. Also demonstrated to exchange personal health records with Microsoft HealthVault.
